FORM 1: RE-AFFILIATION TAX INVOICE

RE-AFFILIATION DUE 01/01/2024 DIRECT DEBIT

On or near 14 January 2024 your Club will be Direct Debited the annual affiliation fee using the banking details supplied to us previously. No receipts will be issued. Please advise us immediately in the section below of ANY change to your club's BSB or Bank Account Number. Due to bank charges for rejected debits due to incorrect details, a fee of $25.00 will be charged to affiliates for non-notification of changes to bank details.

Affiliation Fee from 01/01/2024 until 31/12/2024 Amount Payable $80.00

Note: Insurance information will be forwarded at a later date.

ONLY RETURN THE SECTION BELOW IF THERE HAVE BEEN CHANGES IN FINANCIAL DETAILS IN THE PAST 12 MONTHS

Special Notice
If you have changed details of your Email address, please notify the Membership Secretary of the change as Account and Certificates of Currency will be emailed before the end of April. Also take the opportunity to complete a form 4 updating your Committee details.

Affiliated members are Incorporated.
It is not a compulsory requirement when forming your Club or joining the GCA.

In the unfortunate circumstances that your Club needs to close, please complete GCA Form 13 and return to the Membership Secretary.
